OFFICE IS CLOSING APRIL 30, 2018

April 24, 2018

The University has let us know that until a patient if registered in their system (Epic), they cannot legally accept medical records.  Many of you have signed releases for us to forward records, but will now need you to let us know when you have registered with Epic so that we may send those.

Please call to register with the University of Iowa Hospitals and Clinics Epic system. The phone number is 866-309-0832.

For scheduling, there is a central Patient Appointment Center number to call regardless of where you will be seeing me, (319) 356-2229.  But if you haven’t registered yet, they will then transfer you to the line above before scheduling.  They are scheduling for the main hospital clinic now.  I expect that by mid-May, they will be booking for the other sites –Johnston Outreach, and the Iowa River Landing clinics.

Thanks again,

We have received an update on my schedule moving forward to the University of Iowa Hospitals & Clinics, Stead Family Department of Pediatrics, Pediatric Pulmonology Division.  My weekly outreach clinic in Johnston will be Thursdays, at 8605 Chambery Boulevard.  This University of Iowa Clinic is about half mile north of Interstate 80/35 on 86th St.  They will start booking those appointments in May.

They are booking Iowa City appointments now, for starting the second week of June.  I will have 2 days per week at the Pediatric Specialty Clinic in the main hospital, and 1 day per week at the Iowa River Landing Clinic (IRL) in Coralville.  The IRL is just off Interstate 80, at Exit 242.

For those who would like to continue to stay with me, I would like you to call (866) 309-0832 to get registered in their medical record system, Epic.  If seeing me in Iowa City, could then ask to be transferred to scheduling Pediatric Pulmonary appointment.  If seeing me in Johnston, would have to call back in May to schedule.

If possible, the sooner that you can get us a signed release, the better we can do at getting your records to the right spot.  For those staying with me, once registered in Epic, we can start to transfer the records into the new system.

There will be about a month when I will not be available, between closing our clinic at the end of this month, and before I am available in Iowa City.  Hopefully the bad viruses will be gone by then, and less likely to have significant problems.  But if needed, it would be necessary to be seen by your primary physician or acute care during that period.

I am expecting that the majority of those who would like to follow with me, will be able to see me either in Johnston on Thursdays or in Iowa City.  It won’t be as convenient, having less options for clinic days, and will almost certainly need to book appointments on schedule, as I expect we will keep those slots full.  We will be working to try to make things go as smoothly as possible, but some things we will still need to work out.  Please let me know if there are major concerns.

I will continue to update our website, www.iowapedsasthma.com, as new information comes in.  It’s not certain if I will still be able to send new notices through the Athena portal after our office closes.

Thanks for your patience.

Scott

It is with deep regret I have to announce Iowa Pediatric Asthma & Allergy will be closing our office effective April 30, 2018. I have accepted a position at the Stead Family Department of Pediatrics, University of Iowa Hospitals & Clinics. There will be more information coming, but I wanted to get the announcement out as soon as possible.

It has been my honor, pleasure, and priviledge working with you. Hopefully, I have served you well. For some of you who are interested, there will be availability to continue to follow with me. The initial plan is that I will be doing an outreach clinic 1 day per week in West Des Moines/Johnston. I have also discussed with the darpartment for those of my patients who still want to follow with me either at the outreach clinig or in Iowa City, we will be able to bring them in as Return Patients, which have a lot more appointment slots availabe than New Patient slots

I hope to have more information as to when and how to schedule appointments for these slots. The most consistent and reliable information regarding our progress will be found on our website. We will also be trying to send out notices through the portal as new information becomes available

For those who will be moving another provider, if the plan is to have your primiary physician manage your care, they should already have receiving the information from each clinic visit. This could also be a recourse for copies for our records for those who need them at a later date. For those moving to another specialist, we will be happy to forward the records at your request. We will need to have this form filled out for a transfer.

Thank you,

Scott Sheets, DO
